DOT Physical Exams:

To undergo a DOT physical exam, please follow these steps:

1. Registration and Appointment Scheduling: Register above and schedule your appointment slot.

2. Download Pass My Physical App: Download the Pass My Physical app onto your mobile device and create an account.

3. Complete DOT Form: Use the app to complete your entire DOT form by following the provided instructions. The initial completion may take approximately 30 minutes, but subsequent updates can be made easily in future years.

4. Clearances: If additional clearances are needed, follow the instructions provided within the app.

5. Form Completion: Ensure all sections of the form, including Personal Information, Medical History, Physical Q&A, are completed. Don't forget to sign your form.

6. Payment: Pay the $100 fee for the exam.

7. Appointment Arrival: Upon arrival at your appointment, select the option to Send your completed form to your provider. (Main Menu > your DOT Form.  Send to your provider.  You will need last 4 of SSN & code to give to me.
